Prior to your first appointment with us, please identify which “Questionnaires Group/s” you fall into and complete the appropriate questionnaires. This will allow your provider to learn as much information as possible to help guide your first office visit so you may get as much out of your time together as possible.

*Review of Systems, Medical Questionnaire, Women’s Questionnaire (women only), Adrenal Questionnaire, Digestion Questionnaire, Thyroid Questionnaire, Acupuncture Informed Consent to Treat
NATUROPATHIC CONSULTS:
* Review of Systems, Medical Questionnaire, Women’s Questionnaire (women only), Adrenal Questionnaire, Digestion Questionnaire, Thyroid Questionnaire, ND Waiver
